Our Services

Skilled Nursing

Experienced RNs & LPNs provide wound care, medication management, IV therapy, and post‑operative monitoring.

Therapy Services

Physical, occupational and speech therapy tailored to restore mobility, function, and communication.

Personal Care

Assistance with bathing, dressing, grooming, and daily living activities while preserving independence.

Speech Therapy

Corrective therapy to develop or re-establish communication skills.

Initiating Service

Typically, physicians initiate referrals from their offices, local hospitals, board and care facilities, skilled nursing facilities. Referrals for home health services may be generated by patients, their families, interested persons and/or other agencies. After our intake coordinator receives the referral information and at the physician’s request, our professional nurse will assess the patient within 24-48 hours, unless specific care is required sooner.

Social Services

Multilingual professional staff assists the clients, their families and caregivers with the traumatic impact of the illness. Linkage with agencies or individuals that offer/provide financial resources and community support groups is also available. These professionals are especially sensitive to the unique multicultural needs of our diverse community.
